Once you have signed up you can look for us as a partner lab using our zip code (63043). Even if you plan to ship a case please sign up for an account and fill out the case information. Once the work authorization is filled out you can print it and send that with your impressions / models.  DDX is able to support all digital files needed for planning a case. Send your DICOM / DCM files in along with your I.O.S STL's if you are using a full digital workflow. There is a PDF below you can print to help step you through submitting a case.
