Functional Esthetics Dental Lab has partnered with Elos Medtech, and Vulcan custom dental milling to bring you a new way to look at implant dentistry.
Elos Accurate® Custom Titanium
Offering you a 2 category 3 tier approach to your implant restorations. Choose Hybrid base or custom abutment, (Zirconia or Titanium} Next choose your finish. Do It Yourself (D.I.Y), Complete or Natural Esthetic.
Elos pricing can only be achieved when using the Elos IO scan bodies. The case has to be sent in digitally. It can be sent in your manufacturer's portal or the files can be attached in our DDX portal.
If the manufacturer's portal is used, we still require that your office creates a case in out DDX portal for case tracking and processing.

Hybrid D.I.Y is just like it sounds, but with a twist. We will design the restoration and mill it for the Elos Ti-base workflow.
You will receive the Monolithic Crown, Ti-Base and Screw. The rest is up to you. Just cement, polish and you're ready to restore. The Ti-Base has a specific timing to help ensure that the timing of your crown to base is correct.

All crowns will be milled from our ZirLife™
pre-shaded zirconia pucks. You can also order a model to help double check your handy work before the patient arrives. Models are an additional cost in this program.

Custom D.I.Y is still an unfinished crown that you will polish in office. The custom abutment can be titanium or zirconia. If ordering a zirconia abutment, you will time the Elos TI-base and cement in office.

Our custom Titanium abutments are gold koted unless instructed otherwise. This helps achieve a nice warm hue for your restoration. All crowns will be milled from our ZirLife™ Pre-shaded zirconia pucks. Models are an additional cost in this program.

We will design the restoration and mill it for the Elos workflow. Our complete crown is a Monolithic or Layered restoration. It cemented when it arrives at your office. Policies & Warranty:
Functional Esthetic ("the laboratory") requires the practitioner or clinic ("the client") have a credit card on file for any category or tier in the Elos Medtech Program ("Program"). Each case in this program will be charged in full for work rendered on the day the case is processed for shipping. If the card is declined the case will not be shipped until invoice is paid in full. The D.I.Y. tier in either category, hybrid or custom, the laboratory is not responsibly for any crown or abutment that is cemented incorrectly by the client. The laboratory is not responsible for any abutment or crown in the D.I.Y tier that fails after case is excepted by the client. This is true unless it is determined that the restoration or abutment did not meet manufacturers specifications.
The Vita Classic shade guide is the shade standard for this program. All crowns in the D.I.Y tier will be milled from our pre-shaded Zirlife™ puck to match the shade selected by the client. The laboratory is not responsible for any incorrect shade selection made by the client in any tier. The client will be charged a new fee for the crown to be reproduced if new shade is requested. This is true unless it is determined that the pre-shaded Zirlife™ puck did not meet the Vita Classic shade guidelines. Once determined , the crown or crowns will be re-milled the same shade as in the previous work order. A model is not included in this program, but can be ordered by the client for an additional cost. The cost per model is $75 for single site and $40 dollars per each additional implant replica analog site. Models will consist of both arches, soft tissue and one Elos PMA analog.
